Causes:

Domain not pointing to server
You have Cloudflare "proxied" (orange cloud) and it is blocking validation
Incorrect or recent DNS change (propagation required)

Solution:

Set Cloudflare to "DNS Only"
Wait for propagation
Try again in 5–15 minutes
